LEGAL NOTICE
NOTICE OF FUNDING AVAILABILITY FOR COMMUNITY MENTAL HEALTH PROGRAMS SERVING THE RESIDENTS OF GENEVA, ILLINOIS
(H.B. 708 FUNDING)
The Geneva Community Mental Health Board is now in the process of accepting grant requests for the 2012 fiscal year. Non-profits that provide services to City of Geneva residents with developmental disabilities, mental illness, and substances abuse could be eligible to receive funding under this statute. The following application procedures apply.
All agencies that request funding from the Board shall furnish at least the following information:
a) a concise statement and description of the agency’s mission and purpose;
b) a complete listing of their board of directors together with a listing of their occupations;
c) a complete listing of all executive staff members of the agency;
d) a description of the population served by the agency, the total number of clients and the total number of clients from within the corporate boundaries of the City of Geneva, Illinois. (For example, if a client lists a mailing address with a house number that includes a directional letter in it, such as 39W100 Stone Creek Road, that client, while served by the Geneva Post Office, is not a resident of the City of Geneva. If a client lists a mailing address, such as 123 Smith Street, that client resides within the corporate boundaries of the City of Geneva.) The actual names and addresses of clients should not be included;
e) the amount of funding requested for 2012;
f) a complete description of how the agency intends to use the requested funding;
g) a proposed method evaluation of the effectiveness of the funding, if received;
h) a copy of the agency IRS, 501(c)(3) not-for-profit designation letter;
i) a copy of the current fiscal year budget and last year's fiscal profit and loss statement showing all sources of revenue and projected expenses; and
j) a completed Summary Application form.
The information provided shall, in a concise manner, adequately and accurately inform the Board regarding the information requested.Applicants are directed not to provide any additional information, unless otherwise requested.
A total of nine (9) copies of the above information should be addressed to:GENEVA COMMUNITY MENTAL HEALTH BOARD, 2012 Funding Request, City of Geneva, 22 South First Street, Geneva, Illinois 60134.
Completed applications must be received at City Hall by 4:30 p.m. on Friday, October 28, 2011, to be eligible for funding. A five minute presentation/question and answer session with the respective agencies will be scheduled on Tuesday, November 8, 2011, beginning at 6:00 p.m. in the Geneva City Hall Council Chambers. The Board anticipates that approximately $139,000.00 of funding will be made available at this time.
