I acknowledge that this constitutes written application for termination of utility service at the address above, pursuant to the Geneva City Code, 9-1-1A. Further, I acknowledge that I agree to pay all charges for services rendered as a result of this request. I understand and agree that failure to pay any amount due to the City of Geneva can result in discontinuation of services until such time as payment has been received.
In compliance with Geneva City Code 9-1-1, I acknowledge that a service deposit is required at the time of application for utility service. Upon termination of all municipal utility services with no outstanding account balance, any deposit balance shall be returned to the customer without interest and will be sent to the forwarding address.
